We can’t wait to have you as a member!  For convenience, we have added our forms to our website.  In order to get your membership cards, you will need to make an appointment to stop by the office and pick them up.  You are welcome to mail or drop off the required paperwork to:

We try and staff our office, but  being a 24/7 fitness center, we do not always have regular business hours.  Please don’t hesitate to set up an appointment to guarantee staffs availability! Forms & Instructions

In order to get you started, we need fully completed and signed KCCC Membership Form, and a completed and signed Par-Q Form for EVERY family member that you want to include in your membership.

Whether you just want the ease of your payment coming directly out of your account every month or you want to receive the benefits of a longer term membership but can’t afford the large upfront cost involved, this could be the option for you!

Just sign up for Monthly ACH Withdrawal and you can receive the special pricing listed in the ($) brackets on our pricing chart!  All we need is the below form filled out and returned to our office with a copy of a voided check.
